Add Your First Patient

Before SOAP Note Buddy can generate notes, it needs to know about your patient. Adding a patient takes about two minutes.

Add a New Patient

1. Open the extension by clicking the SOAP Note Buddy icon.
2. Click the + button at the top of the patient list.
3. Enter the patient’s name.

Provide an Eval Summary

The eval summary is the foundation for every note the AI writes. It should include the patient’s diagnosis, relevant history, goals, precautions, and baseline functional status.

You have two ways to provide it:

Option A: Type or Paste

Type or paste your eval summary directly into the eval summary field. You can pull this from your initial evaluation or plan of care.

Option B: Upload a PDF

1. Click Upload PDF and select the document.
2. Click PHI Scrub to automatically remove protected health information (names, dates of birth, addresses, etc.) from the text.
3. Click AI Summarize to condense the document into a structured eval summary.
4. Review the result and edit as needed.

Save the Patient

Click Save to add the patient to your list. You can always edit the eval summary later by tapping the patient’s name.

Tips for Better Results

• More detail means better notes. Include specific goals, measurements, and functional limitations.
• Include all active goals. The AI uses these to match content to the correct goal sections in your EMR.
• Update as goals change. Keep the eval summary current for the most accurate output.
