Add Your First Patient

Adding a patient takes a few seconds. All you need is a name. Adding a patient lets the AI build on that patient’s history over time.

Add a New Patient

In the extension:

1. Open SOAP Note Buddy. It opens on the Visit screen.
2. Click the patient switcher at the top and choose + New Patient.
3. The Add Patient form opens. Enter the patient’s name.
4. Optionally add a patient summary or use Add Context to attach a document.
5. Click Save.

In the app, tap Select Patient on the Visit screen and choose + New Patient. The same Add Patient form opens. Enter the name, add a summary or context if you like, and tap Save Patient. You return to the screen you started from with the new patient selected.

Only the name is required. You can start generating notes right away.

The Patient Summary (optional)

The patient summary is background that stays true across visits: diagnosis, relevant history, goals, precautions, and living situation. It is optional, and you can add or edit it at any time. Add it on the Add Patient form when you create the patient, or later: open the patient list on the Visit screen and click the edit icon to the right of their name.

You do not need it to generate a note. Every note you write also builds the patient’s history automatically, so context accumulates as you document. Add a summary when you want the AI to know something that will not show up in a single visit, such as a weight bearing precaution or a long term goal.

Keep the patient summary for background, and put what happened today in the visit summary instead. Those are two different fields with two different jobs.

Add Context From a Document

If you have an evaluation, plan of care, or intake form, use Add Context to attach a PDF, document, or image. The AI reads it and uses the information alongside your visit summary. Uploading images requires the Scribe plan. PDFs and documents are included in every plan. Add Context opens from the edit icon to the right of the patient’s name in the patient list.

Privacy

Identifiers such as names, dates of birth, and addresses are removed automatically before anything is saved or sent for processing. You do not need to scrub text yourself.

Tips for Better Results

• The visit summary matters most. It is what each note is generated from.
• Add detail when it helps. Specific measurements and functional limitations produce more specific notes.
• Update the summary as goals change, so long running context stays accurate.
