Wrong or Inaccurate Content
If SOAP Note Buddy is generating content that does not match your expectations, the most effective fix is giving the AI better input. The quality of generated notes depends directly on the detail in your patient summaries and instructions.
Improve Your Patient Summary
The eval summary is the foundation the AI uses for every note. A detailed eval summary produces better results:
- Include the patient’s diagnosis, treatment goals, and relevant history
- Note functional limitations and baseline measurements
- Mention the therapy setting (home health, outpatient, SNF, etc.)
The more clinical context you provide, the more accurate and specific the generated notes will be.
Write a Detailed Daily Summary
Before generating notes, enter a daily summary describing what happened during the visit:
- Specific exercises performed and how the patient responded
- Observations about progress, tolerance, or setbacks
- Any changes to the treatment plan
- Measurements or assessment results from the session
A vague summary like “patient did exercises” will produce vague notes. A specific summary like “patient performed standing balance exercises for 10 minutes, demonstrated improved single-leg stance to 15 seconds, reported decreased pain with ambulation” will produce detailed, accurate notes.
Use Custom Instructions
Custom instructions let you control the AI’s writing style:
- “Keep notes concise, 2-3 sentences per field”
- “Use formal clinical language”
- “Always mention functional outcomes”
- “Include specific measurements when provided”
- “Focus on skilled interventions and patient response”
You can set custom instructions per patient or as a global default in Settings.
